Minnesota offers several health care programs to people who qualify.  You can apply for any of these programs with the same application - the Minnesota Health Care Programs Application.
 
You may fill out and print an online application at:  https://edocs.dhs.state.mn.us/lfserver/Public/DHS-6696-ENG
 
You may also pick up an application at Morrison County Social Services or call us at 320-632-2951, toll-free at 1-800-269-1464, to obtain a Health Care Program Application.
