MNChoices Assessment: services to help seniors makedecisions about long-term care with the goal of people remaining in the community. Includes early intervention visits, pre-admission screenings prior to moving into a nursing home, information about public and private programs that can pay for services, transition assistance to help people move out of nursing homes and access community-based services.
Care Coordination / Case Management: provided to certain public program recipients. Includes assisting in locating and gaining access to medical, social, educational, or other needed services, regardless of the funding source. Coordinating the plan of care, monitoring services to maximize health and safety, and eligibility reassessments.
