MnCHOICES Assessments

MnCHOICES Assessment:services help senior’s make decisions about long-term care with the goal of people remaining in the community. Services include:

  • Early intervention visits
  • Pre-admission screenings prior to moving into a nursing home
  • Information about public and private programs that can pay for services
  • Transition assistance to help people move out of nursing homes
  • Access community-based services.

Care Coordination / Case Management

Care Coordination / Case Management services are provided to certain public program recipients. Includes assisting in locating and gaining access to:

  • Medical
  • Social 
  • Educational 
  • Other needed services, regardless of the funding source.  
  • Coordinating the plan of care, monitoring services to maximize health and safety, and eligibility reassessments.
